Please note: completion and submission of these documents does NOT guarantee enrollment with our schools, an individual meeting with the school’s director/administrative staff is also required.
You must complete and review the Potential Student Information Sheet with the school’s administrative staff PRIOR to completing any of the forms listed on this page. Please contact the school directly to do so.
In addition to the forms and documents listed on this page, the following documents are also required PRIOR to official enrollment at our Summit Academy schools:
- a copy of the birth certificate
- immunization records if entering KG, 7th, or 12th
- proof of residency (dated within 60 days of enrollment)
- court documents (if applicable)
